1. Report member’s deaths in a timely manner on the
three part form in the program book or report on line on the interactive
report form that is sent directly to Renee Fulk, Department Chaplain. Be
sure to report to your District Chaplain also.
2. The Official Visit Report from the District
Historian is due to the Department Historian within two (2) weeks after the
end of the Department President’s visit to your District.
3. The Auxiliary History Books are due to the
District Historian within 2 weeks after the 2010 Auxiliary Installation.
4. One winning History Book from each District shall
be submitted at the last Council of Administration in June of 2010, before
the 2010 Convention starts.
5. Special Memories of Sue Wells, Department
President can be submitted to the Department Historian any time.
6. Submissions for Hospital Volunteer Pins (both
Department and National) for Auxiliary and Junior Girl members should be
sent as soon as they are earned.
7. Hospital Volunteer Recruitment forms should be
submitted as they are earned. Do not hold these forms.
8. Forms for membership pins should be submitted as
earned. When you fill out the membership application with the prospective
new member, be sure to put your name on the back as ‘recruited by’, so that
the membership pin can be confirmed.
9. Medical Assistance forms and Cancer Grant forms
should be submitted in a timely manner, as needed.
